A good registration form asks for what you need to run the event and nothing else. These are the six groups of questions to choose from. For wording you can copy, see our list of event registration questions.
Full name and email address, always. Add a phone number only if you will call or text people about changes on the day.
The ticket type, session or time slot, and how many places. For group bookings, ask one person for the names of everyone in the group.
Dietary requirements, accessibility needs, T-shirt size or room preference. Ask only when the answer changes what you order or prepare.
An emergency contact, medical notes and a liability waiver for sports, camps, retreats and anything overnight.
One optional question, such as how the attendee heard about the event. Keep it to one so it does not slow the form down.
Ask separately for permission to take photos and to send future emails. Leave both checkboxes unchecked by default.
Choose from 10 event registration form templates. Select one to load its suggested fields, then edit them for your event.
Capture who is coming and who they work for, so badges, catering and sponsor reports are right on the day.
Ask about experience and goals up front so you can pitch the session at the right level.
Keep it short for repeat students, with an emergency contact for anything physical or in person.
Collect the details you need for seating and catering, plus the organization behind each guest.
Count the whole household in one registration and find out who needs childcare and who can help.
Cover safety first: date of birth, emergency contact, medical notes and a waiver checkbox.
Plan rooms, meals and arrivals before anyone travels, and know who to call if something goes wrong.
Ask for as little as possible: name and email, with optional questions about work and topics for the speaker.
Find out who is in the room and what they are looking for, so you can make introductions.
A light RSVP: who is coming, what they eat, and anything the host should know.
Select “Use form” and create a free account in Checkout Page, our event registration software. Your page opens with the same fields, ticket price and look.
For a paid event, connect Stripe to sell event tickets on the same page as the form. Add more ticket types or a capacity limit if you need them.
Send the link by email, post it on social media, or embed the form on your website. Each registration gets a confirmation email and a ticket with a QR code for check-in.
